الإدمان كالدين لا يمكن سداده

هناك أنواع مختلفة من الإدمان. يمكن أن تظهر في أي عمر بعد أزمات الحياة وتأثيرها المؤلم على الفاعل. عندما تتجاوز الأحداث الداخلية أو الخارجية قدرتنا المعتادة على مواجهة أي نزاع ، فإننا نميل جميعًا إلى تناول الطعام والشراب والدخان أكثر من المعتاد، بالاضافة إلى تجاوز الأدوية أو الاندفاع نحو العلاقات من أجل النسيان المؤقَت. يتبين أن هذا الاقتصاد النفسي لا يمثل مشكلة إلا عندما هو يظل الحل الوحيد المتاح للفاعل لتحمل الألم النفسي.

الإدمان ليس النظام مثل العصاب أو الذهان أو الانحراف الجنسي. يعتمد التعريف الكلينيكي على ثلاثة معايير:

– التكرار الإجباري ؛

– استمراره على الرغم من عواقبه الضارة ؛

– .الهوس به

يعطي الإدمان رغبة لا يمكن كبتها في تناول المادة (الاندفاعاية ) مع التزام التكرار ( القهرية ). يبدأ الإدمان عندما يهدد الخطر، الذي يسميه الصدمة الثانوية، الأنا و آليات الدفاع . في الواقع ، كان الإدمان موجودًا سبقًا ولكنه كان مقبولًا اجتماعيا (على سبيل المثال، الإدمان على العمل أو الرياضة) ، أو كان لدى الشخص العلاقة الحميمة التي كانت تسانده ( مثلا، الزوج في أغلب الأحيان) و اسمه العلاقة الاستنادية، ولذلك لم  يلاحظه اي أحد هذا الإدمان. عندما يظهر تهديد لأنا عقب فقدان القاعدة الاستنادية (التقاعد) أو العلاقة الاستنادية (وفاة الزوج)  ويظهر الإدمان نفسه. إن الإدمان هو مشكلة النرجسية الأولية، خاصة الفشل النرجسي الذي عاشه الفاعل في البيئة الأبوية.

يشكل البحث القهري عن الموضوع المثالي جوهرا الفاعل المدمن. انواع منتشرة للإدمان هي:

– الكحول ،

– المخدرات،

– التَبٌغ ،

– الأدوية،

– الطعام،

– التسوق القهري،

-ألعاب القمار

– الرياضة،

– الجنس ،

– العمل،

– التلفاز،

– الإنترنت ،

– التليفون المحمول،

– محاولات الانتحار ،

– الاضطرابات النفسية التي تؤدي الى الجنوح (مثلا، تكرار قتل الاشخاص الابرياء)

الفاعل المهدد بتدمير ذاته، يسعى إلى إعادة التأمين النرجسي الذي يفتقر إليه خارجيا. عند الشخص البالغ ، يتم تمثيل الاشخاص والموضوعات الاستنادية التي تغطي النقص، من قبل أفراد الأسرة ، والأطفال ، والازواج ، والأصدقاء ، والحياة المهنية ، والهوايات المختلفة .هي تعوض الفاعل عن نواقصه داخلية. وكل شيء يسير على ما يرام طالما تعمل هذه الموضوعات الاستنادية. عندما تنهار هذه الموضوعات الاستنادية (توقف الحياة المهنية، وفاة فرد أو أكثر من أفراد الأسرة) فإن يعاود ظهور التهديد الداخلي  ويعاد الإدمان بهدف وحيد و هو معالجة هذه المعاناة التي لا يمكن السيطرة عليها. يعوض الإدمان عن فقدان السند الخارجي. ومن هذا   المنطلق لديه وظيفة الحفاظ على البقاء النفسي للفاعل. بطريقة ما يسمح موضوع الإدمان للفاعل الا يصبح مجنوناً. من المناسب التفكير في هذا الأمر قبل القضاء على مادة الإدمان باعتبارها أسوأ شر. لا يقع التكرار  على جانب المعنى بل على جانب الاستمتاع الذي يملأ النقص. التكرار هو لم الشَمْل مع الموضوع في الواقع.

يمثل الإدمان عيادة الفعل وليس التفكير. موضوع الإدمان هو موضوع الوجود الخالص الذي يرمز إلى الغياب غير مرموز. الإدمان هو الدَيْن لا يمكن سِداده. لذلك، يصبح الجسم بديلاً للديون غير المسددة. يعمل الإدمان كدافع جزئي: الفعل، التكرار، الجَشَع والاعتماد. لا شك فيه، أن الإدمان يشبه الانحراف الجنسي. لا ينظر إلى المدمنين على أنهم منحرفين لكنهم يرفضون الإخصاء الرمزي ويستمتعون بالمادة مثل المنحرفين. يرتبط الإدمان بفشل الإخصاء الرمزي، مما يترك الفاعل مع القلق لا يمكن تنظيمه رمزياً.  يعتمد الإدمان على وهم حصول جنة مفقودة الطفولة.

لا ينبغي أن يركز العلاج النفسي فقط على سلوك الإدماني ، ولكن أيضًا على الشخصية والتجربة الذاتية لضحية الإدمان. إن خصوصية علاج المدمن هو أن لديه أسباب اخرى لزيارة المحلل النفسي بخلاف الإدمان نفسه. فالسبب هو اللغُز الذي يحمله الفاعل. يجب على المحلل أن يتساءل عن موقف الشخص في إدمانه والاستمتاع به. يبدأ العلاج عندما يصبح الإدمان أحد الأعراض، أي عندما ممكن استجوابه، ليس كسلوك ولكن كتجربة الاستمتاع، كنوع من الأَلَمِ. هناك يوجد التاريخ المزدوج، بما فيه تاريخ الشخص وتاريخ إدمانه. بالنسبة للتحليل النفسي ،فعنده مسألتين: المسألة الأولئ: الانتقال من الإدمان إلى التاريخ الحقيقي للفاعل و المسألة الثانية هي انفصال الفاعل عن إدمانه (و ذلك يعني سماع المدمن بطريقة مختلفة عن ضحية الإدمان). يظهر الإدمان، مثل اية الاضطرابات الأخرى، كنتيجة لجهود الأنا للتكيف مع الإحباط والمطالب الاجتماعية. المدمن هو عبد الحل الوحيد و هو الهرب من الألم النفسي. في رأي المدمن ان ما يدمنه هو شي مفيدالذي يمكن أن ينتهزه في أي لحظة لتخفيف الحالة العاطفية التي لا يمكن تحملها. أحد أهداف السلوك الإدماني هو التخلص من التأثيرات العواطفية وجعل الصعوبات اليومية مٌحتَملة. يَبْني المدمن الهَوِية الجديدة، وهو الحل الوهمي والمتناقض لاستعادة الهوية حيث تكون الموضوعات الانتقالية متاحة دائمًا، ويمكن التحكم فيها تمامًا، وبالتالي تحقيق حالة مثالية من الرضا والاكتفاء الذاتي. ليس من السهل على الإطلاق التخلص من الإدمان لأنه يحمل وظيفة نرجسية. موضوع الإدمان يعطي شعورا بأن الأنا يَنْجي بطريقة ما، فالأنا يتحكم على الأقل في شيء في حياته.

Addiction as unpaid debt

There are different types of addiction. It can appear at any age following the life crises and their traumatic effect on the subject. When internal or external events exceed our usual ability to cope with a conflict, we all tend to eat, drink, smoke more than usual, as well as overtake medication or to rush into relationships for sake of momentary forgetfulness. This psychic economy turns out to be problematic only when it remains the only solution available to the subject confronted with unbearable psychic pain. 
Addiction is not a structure, like neurosis, psychosis or perversion. The contemporary clinical definition of this dependence is based on three criteria:
– the compulsive repetition;
– its persistence despite its harmful consequences;
– the obsession with it.
The addiction gives the irrepressible desire to consume (impulsive dimension) with obligation of repetition (compulsive dimension). Addiction begins whenever a hazard of life, called secondary trauma, threatens the I and jeopardizes the defense mechanisms. In reality, the addiction pre-existed but was either socially acceptable (addiction to work or sport), or supported by a reassuring anaclitism (with the spouse most often) and therefore did not appear and went unnoticed. It is only with the loss of the supportive base (retirement) or the anaclitism (death of the spouse) that the life threat for the I appears, and addiction manifests itself. It is the problematic of primary narcissism, in particular the narcissistic failure experienced in the parental environment.
The compulsive search for an ideal object lies at the core of the addicted subject. The most popular addictions are:
– alcohol,
– drugs,
– tobacco,
– medication,
– food,
– compulsive shopping,
– gaming
– sport,
– sex,
– work,
– television,
– internet,
– mobile,
– suicide attempts,
-psychopathy of delinquency.
The subject threatened with the collapse of his unity, seeks the narcissistic reinsurance which is lacking within him externally. In the adult, the supportive figures and objects, which cover the lack، are represented by family members, children, partners, friends, professional life, and various hobbies. They compensate for the internal deficiencies of the subject and everything goes well as long as these supportive objects function. It is only when they collapse (cessation of professional life, death of one or more members of the family, etc.) that the internal threat reappears and addiction is brought into play with a sole aim of healing this unmanageable suffering. Addiction compensates for the loss of external support. In this sense it has the function of preserving the psychic survival of the subject. The object of addiction, in a way, allows the subject not to go crazy. It is appropriate to think about it before eradicating the object of addiction as the worst evil.
Repetition is not located on the side of sense but on the side of enjoyment, which fills in the lack. Repetition is a reunion with the object in reality. Addiction is a clinic of acting, not thinking. The object of addiction is an object of pure presence which symbolizes absence but which is not symbolized. The addiction is a debt that could not be paid. Therefore, the body becomes a substitute for unpaid debt. Addiction functions as a partial drive: act, repetition, greed and dependence. Undoubtedly, addiction has the most things in common with perversion where the sexual occupies only a small space, that is to say, desexualised perversion (with the exception of “sexual addictions” disorder). Not that addicts are seen as perverts, but the theory of perversion makes it possible to question the relationship of the addiction with the denial of castration and the enjoyment of the instrument. Addiction is linked to the failure of symbolic castration, which leaves the subject with anxiety that can not be symbolically organized. The dependence is linked to the illusion of finding the lost paradise of childhood. Therapy should not focus solely on addictive behavior, but also on comorbidity, personality and the subjective experience of victim of addiction. The specificity of the therapy for an addicted is that addiction is often perceived as the reason for seeing an analyst but it may not be its main and sole reason. The cause lies elsewhere rather than in the addiction itself, that is to say in the mystery, which the subject preserves. Analyst should question the subject’s position in addiction and his enjoyment.
The cure begins when the addiction becomes a symptom, that is to say when it can be questioned, not as behavior but as identity, as experience of jouissance, as modality of the pain. There exists a double history, that of the subject and that of his addiction. For psychoanalysis, the issue is twofold: to go from addiction to real historization of the subject and to separate the subject from his addiction (which means hearing an addict differently than a victim of an addiction). Addiction appears, like other disorders, as a result of the ego’s efforts to adapt to frustration and social demands. The addicted subject is the slave of a single solution to escape the mental pain. The object of addiction is invested with beneficial qualities, even with love: an object of pleasure, which he can seize at any moment to attenuate affective states experienced as intolerable. One of the aims of addictive behavior is to get rid of one’s own affects and to make everyday difficulties bearable.
An addict constructs a neo-identity, an illusory and paradoxical solution of restitution of the identity where transitional objects are always available, completely controllable, thus fulfilling an ideal state of satisfaction and self-sufficiency. Object of addiction gives feeling that I copes up somehow, I is in control of at least something in his life. It is not easy at all to get rid of addiction since it holds a narcissistic function.
